The purpose of the implant
The penile prosthesis is designed to provide the rigidity required for sexual activity in certain cases of erectile dysfunction. It should not be presented as a lengthening procedure or as a guarantee of an increase in size.
Distinguishing between device expansion and anatomical outcome
Some devices expand during use. This technical feature does not mean that the resulting length will exceed the available anatomical length. A perceived loss of length following implantation is possible and should be discussed prior to the procedure.

Setting realistic expectations
Ask the surgeon how they assess length prior to the operation, what measurement is used as a reference, and what factors specific to your situation may influence the outcome. Medical history and the condition of the tissues are more important than a general marketing claim.
Avoid choosing a model solely on the basis of an advertised gain in centimetres. Also ask what expectations are realistic regarding rigidity, the glans, sensitivity, orgasm and ejaculation: these functions are not limited to the mechanical operation of the implant.
After the operation
Learning how to handle the implant and any necessary rehabilitation must follow the surgical team’s instructions. Do not attempt to increase the size using unauthorised techniques or devices.
Questions to ask
What outcome can reasonably be expected in my case? How will it be assessed? Who should I contact in the event of pain, difficulty with handling the implant or dissatisfaction? What are the limitations of the device and the conditions under which a revision operation might be necessary?
